Patient Screen ShotCard™ and ShotCardPro™

The below screen shot is the Patient screen in ShotCard™ and ShotCardPro™.  Fields labeled in red are required fields. Note the
immunization button, dose certification button (printed shot card list) and the more information button. The more information
button allows for recording of health history, public health data and insurance data. The dose certification button will produce a
printout of dose history for presentation to public schools or other entities. The immunization button takes you to the Encounter
screen, Encounter Screen ShotCard™, where new doses may be added and recommended doses may be seen.
